L'infermiere case manager specializzato nelle cure domiciliari opera all'interno della Centrale Operativa di Medicasa e rappresenta il punto di riferimento primario per il paziente, la famiglia (caregiver) e l'équipe multidisciplinare. Facilita e ottimizza i rapporti e la comunicazione con i Medici di Medicina Generale (MMG), i Distretti Sanitari e gli operatori del territorio.…
