This role is designed for a dedicated nursing professional who is passionate about delivering high-quality patient care and enhancing patient outcomes through effective case management practices. The Case Manager is responsible for the coordination of care and service delivery for an assigned group of patients and promoting interdependent communication and collaboration with the patient, caregiver, physician, and other members of the health care management team.
