Ambulatory Care Clinical Pharmacist Responsibilities: Works under collaborative practice agreements with physicians, advanced practice nurses, and physician assistants to manage patient therapeutic needs related to hypertension, hyperlipidemia, diabetes and other chronic conditions as needed. Collaborates with physicians, advanced practice nurses, physician assistants, medical assistants, dieticians, care coordinators and others to address patient treatment needs related to hypertension, hyperlipidemia, diabetes and other chronic conditions.
