Intake Evaluator Case Therapist Northwestern Medicine Location: Winfield, Central DuPage Hospital, Behavioral Health Services Schedule: Monday-Friday 1:00 PM-9:00 PM Responsible for assessing patients for all levels of mental health and substance use concerns. Assessments will occur in outpatient behavioral health setting, emergency department and medical/surgical units. Responsibilities include treatment planning, providing direct patient services, to include on a limited basis individual and family counseling if needed. Case Therapist coordinates care including communicating with treatment team, patient, and family, documenting in medical record, and discharge planning, continuum of care.
