Treat patients via telehealth visits, including video visit appointments, and message-based care Manage patients with a broad array of patient needs virtually, conducting a mix of acute, chronic, and well visits Adopt standard work and clinical protocols for evidence-based clinical care Continuous learning during weekly Clinical Rounds and through other modalities Ongoing collaboration with teammates and leadership via daily huddles Collaborative oversight of 4-8 NP or PA colleagues Education, licenses and experience required for this role: Completed an accredited Internal or Family Medicine residency program Practiced at least 2 of the last 5 years in a primary care or virtual primary care setting Board Certified in Internal or Family Medicine Active state license in either CA, NY or DC…
