KeystoneCare is seeking an experienced and highly organized Home Health Admissions Registered Nurse (RN) to join our team on a Full-Time or Part-Time basis. The Admissions RN is responsible for completing comprehensive home health admissions and ensuring patients have a safe, accurate, and timely start of care. This position is ideal for an RN who has strong clinical assessment skills, is comfortable working independently in patients' homes, and can manage admission documentation efficiently. Key Responsibilities Complete comprehensive and accurate Home Health admission and Start-of-Care (SOC) assessments. Evaluate skilled needs, eligibility, homebound status, and admission requirements.
