Northwestern Medicine has an exciting opportunity for either a Acute Care Nurse Practitioner to work with our Hospice team. This position will be 100% inpatient. The APP will provide comprehensive hospice and palliative care services across inpatient and community settings, including face-to-face hospice visits, symptom management, eligibility assessments, inpatient rounding for new GIP admissions, and active participation in interdisciplinary team (IDT) meetings. Schedule: Monday-Friday, 11:00am-3:00pm Call required.
