Northwestern Medicine has an exciting opportunity for either a motivated Acute Care Nurse Practitioner or Physician Assistant to work with our Palliative Medicine team. This APP will work collaboratively within an interprofessional team of physicians, advanced practice providers, nurses, and social workers to provide comprehensive, compassionate patient care. Participates in rotating inpatient coverage and shared call responsibilities, advocates for patients and families, collaborates with the broader care team, and assists with goals-of-care discussions and complex care planning. This role include both inpatient and ambulatory responsibility.…
