Accord is seeking a mission-driven Waiver Case Management (WCM) to join a team of Case Managers in making a difference in the lives of people living with disabilities. The WCM will evaluate, create, and support the health and human service needs of people that Accord serves. This is a hybrid role with offices located in Midway St. Paul, MN. The WCM will be expected to travel within the community to meet with supported persons. Staff will work on-site during training and orientation. $1500 Hiring Bonus paid over one year!
