A Day in the Life The Behavioral Health Care Manager (BHCM) is a nurse navigator who plays a key role in Munson Healthcare’s Collaborative Care (CoCM) program. CoCM is an evidence-based model that integrates behavioral health services into the primary care setting through a team-based, measurement-driven approach and proactive patient follow-up. Working in partnership with the patient, Primary Care Provider (PCP), and Psychiatric Consultant (PC), the BHCM provides care coordination and behavioral health support for patients experiencing depression and anxiety. This role is similar to care management for chronic physical health conditions and helps strengthen communication, continuity, and collaboration across the care team.…
