The Signature Services Clinician reflects the mission, vision, and values of Northwestern Memorial, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. Responsibilities: Takes patient vitals. Greets incoming patients. Acts as a liaison between the Program and other hospital departments/physician offices. Escorts the patient throughout the day to meet their needs. Trouble shoots and problem solves care delivery issues. Notifies clinic leadership and/or appropriate department/physician office of deviations in the patient’s schedule. Meets needs of patients who come through the Program. Assures that patient arrive promptly at scheduled appointments.…
