**This is a full time salaried position at 40 hours per week located at our Lake Forest Outpatient Clinic. Clinicians are expected to work two shits per week until clinic close at 7p. ** The Physical Therapist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. A licensed physical therapist who complies with the Code of Ethics/Standards of Practice as outlined by their professional organization and by the State of Illinois.
