About this job
This position would assist with covering needs at the five NM Cancer Centers in Orland Park, Oak Brook, Warrenville, Geneva, and DeKalb M-F no nights, weekends, or holidays. Assists with covering scheduled days off and leaves as applicable at least two shifts per month. Typical general hours would be 8a - 4:30p. The Social Worker reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. Responsibilities: Collaborates and participates in the coordination of screening, assessment, and discharge planning activities in high risk patients and their families with complex psychosocial and economic needs.…
